Skilled Nursing
Teaching of the patient or family, disease process, monitoring blood pressure, pulse, medications and wound care. Assessing the patient wound, body systems related to disease. Services that require a skilled nurse, such as, foley changes, IV, IM medications.

NON Skilled Services
Daily blood pressure monitoring, daily weight measurements, insulin injections.

Medical Social Worker
Advance directive, evaluate financial status for community assistance, CBA program, Homemaker programs, meals on wheels. Help with dysfunctional family dynamics.

Speech Therapy
Post DVA, Dysphasia swallow studies.

Occupational Therapy
For the upper body, fine motor skills, ADL training, cooking

Physical Therapy
Gait training, strengthening, mobility, transfers.

Aide
Personal care assistance with bathing, dressing, shampoo, teeth care, and incontinent care.
